Forms are not available online at this time.   After completing form 137 and the Finger-Print form (have your fingerprints taken at your local police station), send your documents to the Embassy of Zimbabwe, 1608 New Hampshire Ave. NW, Washington, D.C. 20009, with a fee of US$40.00 in money order and a self-addressed, stamped envelope for the return of your documents. Processing time is 5 working days.   Upon receiving the processed documents from us, deposit US$75.00 into the Officer Commanding Criminal Investigation Department, Standard Chartered Bank, Highlands Branch, Account No. 0100216046400 (Zimbabwe).   Attach the telegraphic slip to the form 137 and the Finger-Print form and forward to:        The Officer-in-Charge      Criminal Records Office      P.O. Box 8125      Causeway      Zimbabwe   If you do not include a return address, the certificate will be sent to this Embassy by Diplomatic Cargo. You will then need to make arrangements with us for the return of your certificate.
